    On September 6th, 2003, the NOAA-19 satellite was being readied at the Lockheed Martin Space Systems factory in Sunnyvale, California.

    During the evening shift, a technician removed twenty-four bolts securing an adapter plate to it without documenting the action. The next moving, the next shift technicians subsequently failed to check the bolts, as specified in the procedure, before attempting to move the satellite.

    As they moved it, the ultra delicate satellite unceremoniously crashed to the floor causing over 135 million dollars in damages...
